Oral Surgery Guidelines – For Hywel Dda and ABMU

Oral Surgery referral Guidelines

 

  • All referrals are triaged to ensure they are suitable
  • This initial triage is to ensure that the requested treatment is suitable for primary or secondary care and that the information provided is correct.
  • Please ensure the forms are filled in correctly as otherwise the form will be rejected.
  • Common reasons for the referral being returned are –
    • Treatment requested does not correspond to charting or the radiograph provided
    • The treatment requested is deemed to be Level 1 – ie suitable for treatment by the GDP based on the information and radiographs provided. The only exception to this is a patient seen on an access session who doesn’t have a regular GDP.
    • Sedation/GA requested but IOSN not filled in. The IOSN needs to be filled in not just attached to the referral
    • No radiographs provided and no justification for lack of radiographs
    • O/S provider that the GDP has selected is not appropriate for the patient or treatment requested (see list below)
    • For TMJ problems please use the Maxillofacial referral form
    • For conditions such as atypical facial pain, atypical odontalgia and other oral medicine conditions (Lichen planus, ROU, white patches, red patches) please use the Oral medicine referral.

 

RECAP OF WHAT IS SUITABLE FOR EACH INDIVIDUAL CENTRE

For patients living within ABMU area (SBU)  – there are only 2 providers with contracts for patients living in this area

 CAMBRIA –

  • All Level 1 & Level 2 procedures
  • LA and IV sedation

PARKWAY-

  • All Level 1 & Level 2 procedures
  • LA and IV sedation for all patients

 If you do not select your preferred primary care provider it will allocate the referral to the nearest oral surgery primary care provider (geographical distance from patient)

If the patient requires a Bariatric chair for treatment and needs a Level 1 extraction the referral will automatically be directed to CDS.

If the patient requires a Bariatric chair for treatment and needs a Level 2 extraction the referral will automatically be directed to Secondary Care (Maxillofacial department)

 If the patient requires a GA the referral will automatically be directed to Maxillofacial department.

For patients that live within Hywel Dda postcodes. There are more providers available for these patients but the main deciding factor in choosing a provider will be geography and the following criteria –

DEW ST (Haverfordwest) –

  • Level 1 and Level 2 procedures under LA alone.

CAMBRIA , Swansea –

  • All Level 1 & Level 2 procedures
  • LA and IV sedation

 PARKWAY, Swansea –

  • All Level 1 & Level 2 procedures
  • LA and IV sedation for all patients
  • GA for suitable adult patients only with suitable treatment plan

NEWTOWN Clinic (Powys)  –

  • Patients with SY postcodes only.
  • Level 1 and Level 2 procedures under LA or GA (No IVS)
  • No Oral Medicine procedures, no biopsies

If the patient requires a Bariatric chair for treatment and needs a Level 1 extraction the referral will automatically be directed to CDS.

If the patient requires a Bariatric chair for treatment and needs a Level 2 extraction the referral will automatically be directed to Secondary Care (Maxillofacial department)

If the patient requires a GA but the treatment requested is complex or the patient has a complex medical history or high BMI the referral will be directed to Maxillofacial department.
